Religious Accommodation Request Form
A religious accommodation is any adjustment to the academic environment that allows students to practice one’s religion, unless such an accommodation would create an undue hardship for the university. To determine whether you are eligible for a religious accommodation under the provisions of Title VII of the Civil Rights Act of 1964, the Office of the Dean of Students will engage in an interactive process to understand the needs of the student. Accommodations vary and are considered on a case-by-case basis.
Upon reaching a final determination at the conclusion of the interactive process, the Office of the Dean of Students will provide the student with a written final determination. As stated in APS 5001 , in accordance with applicable federal and state law, and Article 8 of the Laws of the Regents , the University of Colorado does not engage in discrimination on the basis of race, color, national origin, sex, age, disability, creed, religion, sexual orientation, gender identity, gender expression, veteran status, political affiliation, or political philosophy.
To request a religious accommodation, please complete a UCCS Student Religious Accommodation Request Form. Upon completion of the form, a representative from the Office of the Dean of Students will contact you within three business days.
